Tubular replacement of the ascending aorta remains a standard and time-tested approach in acute aortic dissection type A (AADA) and absence of preexistent root aneurysm. At times, persistent dissection of the root may contribute to aortic valve failure, requiring reoperation. If valve failure is present, the valve will frequently be placed. Experience has shown that the reoperation can be performed preserving the native valve. This video shows a case in which aortic regurgitation led to the need for reoperation 6 weeks after the initial procedure. The operation included replacement of the aortic root for better valve stabilization. The technical details are highlighted.
