All forms of valve preserving surgery require that the preserved or reconstructed aortic valve will have normal or near-normal form. Adequate commissural height and configuration are an important prerequisite for valve form; inadequate height can lead or contribute to valve failure. If commissural height is insufficient, the valve can still be preserved, but the inadequacy must be corrected by undoing the original procedure and repeating it with adequate geometry. This video demonstrates the reoperation of a failed reimplantation procedure. In order to achieve adequate aortic valve form, the graft inserted for reimplantation was removed completely and the procedure was redone from beginning, in this case as remodeling procedure.
